Completing the Student Intake Form is the first step in the registration process.

All information supplied is kept confidential and submitted directly to the SSD office.
Upon submission, you will be contacted via email by the SSD office within five (5) business days. Communication will be sent directly to the student's University of Michigan (UMICH) email address. If you have not heard from us after that time, please contact our office at 734-763-3000.
Please do not send in documentation until it has been requested.
Due to COVID-19, our physical office space is currently closed and our staff is working remotely.
At this time, documentation cannot be faxed, mailed, or dropped off at this time.
The University recognizes that student disability records contain confidential information and maintains such confidentiality in compliance with applicable law. Any information collected is used by SSD for the benefit of the student and/or in order for SSD authorized staff to fulfill their professional responsibilities. No one has immediate access to student files at SSD except authorized SSD staff. Under the Family Educational Rights and Privacy Act (FERPA), SSD may disclose your education records, including student disability records, with other University Officials only if a legitimate educational interest exists that mandates the sharing of information. . In general, University faculty and staff do not have a right or a need to access diagnostic or other information regarding a student's disability. When it is necessary, faculty and staff will be informed of the functionally inhibitive manifestations of the student's disability and the accommodations necessary or appropriate to meet the student's needs. When a student requests an accommodation, he/she/they will be informed as to what information will be provided to the faculty or staff regarding the request.
To provide your written consent to disclose disability-related information to third parties outside the University, which could include your parents or other individuals that you specify, you must submit a release form, found online or in the SSD office.
